Welcome to the Guelph Wellington Ontario Health Team


The Guelph Wellington Ontario Health Team (OHT) is releasing their 2022-2025 Strategic Priorities focusing on how to improve health care through our collective work. The priorities were identified through consultation with our community, health, and social service providers. The priorities have also been endorsed by the Guelph Wellington Patient Family Caregiver Advisory Council.

The three priorities include:
1) Stronger integration and communication
2) Improved health equity
3) Pandemic recovery

Residents of Guelph and Wellington will be some of the first in Ontario to benefit from a new model of organizing and delivering health care that better connects patients and providers in their communities to improve patient outcomes.

On Nov. 29, 2019, the Honourable Christine Elliott, Deputy Premier and Minister of Health, announced the Guelph and Area Ontario Health Team (OHT) would be among the first 24 teams in the province. In June, 2021, our OHT was expanded to include partners from Rural Wellington and renamed the Guelph Wellington OHT.

In the Guelph Wellington area, we are building on what we already do well – collaborating to meet the needs of patients. Together, we are able to achieve much more than we can separately. All of us are committed to strengthening our local health system and available services, improve client experiences and ultimately, make the community healthier. While OHTs are being established, community members can be assured their existing health services will remain available and accessible.
