GUELPH WELLINGTON ONTARIO HEALTH TEAM STRATEGIC PRIORITIES 2022-2025
The Guelph Wellington Ontario Health Team (OHT) 2022-2025 Strategic Priorities (described below) are the collective focus of OHT partners to collaboratively improve the health of persons living in Guelph Wellington. The priorities were identified through consultation with our community, health, and social service providers. The priorities have also been endorsed by the Guelph Wellington Patient Family Caregiver Advisory Council. The image on the left describes the benefits of achieving our strategic priorities.
The three priorities include:
1) Stronger integration and communication
2) Improved health equity
3) Pandemic recovery

In the Guelph Wellington area, we are building on what we already do well – collaborating to meet the needs of patients. Together, we are able to achieve much more than we can separately. All of us are committed to strengthening our local health system and available services, improve client experiences and ultimately, make the community healthier. While OHTs are being established, community members can be assured their existing health services will remain available and accessible.
