GUELPH WELLINGTON ONTARIO HEALTH TEAM (GW OHT) RELEASES MID-TERM REPORT SHOWCASING PROGESS ON STRATEGIC PRIORITIES

The Guelph Wellington Ontario Health Team (OHT) is pleased to share the progress towards improving the health of residents and patients in Guelph and Wellington County.
The Guelph Wellington Ontario Health Team (OHT) is at the mid-way mark of its 2022-2025 Strategic Priorities and has released a report on the progress to date. The GW OHT is comprised of partners who have made commitments to work collaboratively to improve the health of the individual.
The GW OHT’s three strategic priorities are: stronger integration and communication, improved health equity, and pandemic recovery. Throughout the mid-term report, there are examples of how the GW OHT partners are working on goals associated with each strategic priority and how progress is being made. Ontario Health Teams (OHTs) bring together partners, including health and non-health organizations, patients, and caregivers in their work as one team to provide integrated care to improve the health of their local population. They share clinical data, use data to support and monitor outcomes and, at maturity, are accountable for a set of outcomes within a defined budget.
The GW OHT partners are committed to creating a fully integrated system of care that feels effortless and meets all the needs of each resident. As partners continue to focus on the strategic priorities, we will work towards a system that functions as one and addresses what matters most to patients, being flexible and responsive to unique health needs. The GW OHT will continue to update on this progress through newsletters, media and our website.
