CBI Home Health and ParaMed Home Health selected as in-home service providers for new integrated home care model in Guelph Wellington OHT
September 27, 2024 – Guelph, ON – Partners of the Guelph Wellington Ontario Health Team (GW OHT) are excited to announce a transformative partnership with CBI Home Health and ParaMed Home Health to deliver home care through existing primary care teams. This innovative partnership marks a significant milestone in advancing integrated primary care in Guelph Wellington, bringing together a comprehensive range of services that allow individuals to receive the care they need from a team they know. GW OHT is one of seven OHTs selected to deliver innovative, modernized home care models with Ontario Health and Ontario Health @Home. This is part of the Ontario Government’s plan to modernize home and community care in the province.
IPCTs are integrated, dedicated teams of providers who work together closely to meet the needs of patients served by existing primary care teams. The innovative Integrated Patient Care Team (IPCT) model brings together primary care, home care, and community providers in a fully integrated, seamless, and team-based approach to meet the unique needs of people served by six existing primary care teams across Guelph Wellington. Patients will experience more personalized, timely, and coordinated care that empowers them to manage their health at home, reducing unnecessary hospital visits and improving overall health outcomes.
“Our partnership with the Guelph Wellington OHT underscores our commitment to delivering exceptional, integrated and patient-centered care,” said Steve Paraskevopoulos, Senior Vice President of ParaMed Home Health Care. “We look forward to co-designing a new model that will streamline access to care and enhance the patient experience.”
“Integrated primary care is the key to unlocking a more responsive and patient-centered healthcare system,” said Jon Hantho, President and CEO of CBI Home Health. “We are proud to partner with the Guelph-Wellington OHT to co-create a new, forward-thinking care model that will meet the evolving needs of our communities and provide better, faster care at home.”
“GW OHT partners are thrilled to be partnering with CBI Home Health and ParaMed Home Health to advance primary care-based integrated care through the Integrated Patient Care Team model, said Emmi Perkins, Director of Transformation with the Guelph Wellington Ontario Heath Team. “Our model is based on high performing health systems around the world where dedicated groups of providers work together to meet the needs of a group of patients in an integrated way. Patients and caregivers told us they need a single team of providers who work together and communicate with each other to help the patient achieve their goals. Providers told us that they don’t always know what care their patients are receiving in the community. In this IPCT model, patients will experience home care and primary care providers working together as a single team. CBI Home Health and ParaMed Home Health bring a wealth of experience and success in delivering integrated care – we can’t wait to get started!”
