The GW OHT Integrated Patient Care Team (IPCT) Home Care Program launched in January 2025. Patients served by 6 existing Primary Care teams in Guelph Wellington now have access to a single, integrated team of interdisciplinary primary care and home care providers (including OH@H Care Coordinators and in-home service providers from Paramed Home Health and CBI Home Health). Building on Guelph Wellington’s strong team-based primary care structures and relationships, these integrated teams provide patients with one team for their primary and home care needs. This is our local solution to modernize home care as part of the Ontario Government’s plan to modernize home and community care in Ontario.
See the impact video below celebrating the success of the GW OHT IPCT Home Care Program and its impact of delivering home care services to our community. As a result of the interventions described in this video, the patient’s A1C decreased from 8.3 to 7.2 between January and April 2025! This occurred with no medication changes but as the result of financial security, medication security, improved mood, decreased stress and food security! (NOTE: A 1% reduction in A1C correlates to a 13% reduction in CV risk). This is a powerful example of how integrated, wholistic, person-centered, primary care-based care that addresses both health and social care needs is having a profound impact on the health of our community.
