The Guelph Wellington OHT is committed to incorporating the experience, input, insights from a wide range of diverse patients, family caregivers and people with lived experiences. The Patient, Family, Caregiver Advisory Council (PFAC) is a group of patient advisors who ensure that the perspectives of persons with lived experience, patients, families and caregivers is present throughout the work of the GW OHT to improve the health of our attributed population. GW OHT working groups regularly engage the PFAC to ensure they are informed, involved, consulted, collaborated with, and empowered.
