Please join us for the virtual Guelph Wellington OHT Hackathon!
Our goal is to explore and identify innovative digital solutions to address a key challenge in integrated care. A hackathon is a collaborative event where participants work together to solve a specific problem—often through creative, technology-driven approaches.
Key Challenge
How can we share care plans seamlessly across members of an integrated care team—composed of staff and providers from different organizations?
Focus Scenarios
- Health and housing providers supporting individuals experiencing homelessness and complex mental health, addiction, and brain injury challenges.
- Primary care providers, Care Coordinators, and in-home service providers (SPO partners) working within Integrated Patient Care Teams (IPCTs).
This session will be recorded and made available for anyone who is interested but unable to attend live.
