Guelph Wellington OHT partners have been collaborating to address our community’s needs throughout the COVID-19 pandemic. Partners have demonstrated the value of community-based leadership and planning in proactively identifying and responding to the unique health needs of all of the residents we serve. In February 2019, a massive overhaul of how healthcare will be delivered in Ontario was announced. Fundamental to the plan is the creation of Ontario Health Teams (OHTs). These are being introduced to provide a new way of organizing and delivering care that is centred on patients, families and caregivers.

Months of planning, community engagement and relationship building have gone into creating the Guelph and Area Ontario Health Team and there’s much more to be done including the recent expansion into becoming the Guelph Wellington OHT. Along the way, we are committed to keeping everyone well informed of the progress being made.
